Dr. Black and his staff would like to sincerely welcome you to our family dental practice in Flushing. We are committed to doing everything possible to provide you with excellent and advanced dental care, while making your visit to our office a comfortable experience.
What to Bring on Your First Visit
Our dental staff tries to provide prompt dental treatment to every patient. In order to keep on track, we ask that you bring the following items when you visit our practice for your initial visit:
- Insurance Information/Card
- Any medical records pertaining to your visit including an up to date medication list
- Photo Identification
- Your new patient forms which were mailed to you
Please arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork.
You may also want to write down any questions or concerns regarding your dental health and bring this list with you. This will help to ensure all your concerns are addressed.
What to Expect on Your First Visit
Your first visit will include the following:
- Review of your complete dental and medical history form
- Review of your dental x-rays and/or taking new x-rays as required
- Dental Cleaning
- Dental Exam

For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards. We are confident that you will feel right at home in our office as we welcome all patients as if they were family.

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete treatment plan will be designed with payment options. Payment is due when services are rendered. Forms of payment accepted by the office are check, cash, or any major credit card.
We are also pleased to offer Care Credit to our patients. With a comprehensive range of plan options, for treatment or procedure fees from $100 to over $25,000, we offer a plan and a low monthly payment to fit comfortable into almost every budget.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
